This guidance from 2009 sets out the circumstances in which health professionals may receive, and respond to, requests for access to health records.

Information Governance

Information Governance ensures necessary safeguards for, and appropriate use of, patient and personal information. Key areas are information policy for health and social care, IG standards for National Programme for IT systems and development of guidance for NHS and partner organisations. GP2GP

GP2GP enables patients' electronic health records to be transferred directly and securely between GP practices. It improves patient care as GPs will usually have full and detailed medical records available to them for a new patient's first consultation. This link will take you to the GP2GP website. 

NHS Care Record Guarantee

The NHS Care Record Guarantee for England sets out the rules that govern how patient information is used in the NHS and what control the patient can have over this. This link will take you to the NHS Care Record Guarantee website.

The Caldicott Committee Report on the Review of Patient-Identifiable Information

This 1997 Review was commissioned by the Chief Medical Officer of England owing to concern about the ways in which patient information is used in the NHS in England and Wales and the need to ensure that confidentiality is not undermined.
